§ 10-114. Service classifications requiring certificates or permits and corresponding fees.

  • (a)

    Classification. Services granted certificates or permits as required under this section shall operate in accordance with the classification of each service category and subcategory as follows and may provide services at any level lower than the certificate or permit issued with the exception of Level V which may only provide air ambulance service under a Level V certificate:


    Level I
    - Nonemergency medical transportation (permit required)
    - Interfacility transport (permit required)
    Level II - BLS interfacility transport (COPCN required)
    Level III - ALS nontransport (COPCN required)
    Level IV - ALS transport (COPCN required)
    Level V - ALS air ambulance (COPCN required)

     

    (b)

    Application fees. Application fees shall be established by a resolution adopted by the board of county commissioners after the initial application fees as provided for in this section. An application fee shall be paid to the county upon submission of an application for service. No application shall be processed until the appropriate fees have been paid. The initial fees shall be as follows:


    Level I
    - Nonemergency medical transportation - $200.00
    - Interfacility transport - $200.00
    Level II - BLS interfacility transport - $500.00
    Level III - ALS nontransport - Exempt
    Level IV - ALS transport - $2,000.00
    Level V - ALS air ambulance - $5,000.00

     

    Local governments shall be exempt from payment of application fees imposed by this article.
