§ 10-117. Procedure for obtaining certificate or permit.

  • Application for a certificate or permit shall be made on forms to be furnished by the county administrator or designee. The application shall contain such information as may be required by the county administrator and the board, and shall include at least the following items:

    (1)

    The names, business addresses and relevant experience of both the operator(s) and the owner(s) of the proposed service. If either the owners(s) or operator(s) are part of a corporate entity, the names and addresses of the directors, officers and controlling shareholders of any such corporations must be provided;

    (2)

    The classification(s) of service, as described in section 10-114, that the applicant wishes to provide;

    (3)

    The area which the applicant desires to serve. Applicant shall include a map that clearly depicts the area to be served;

    (4)

    A statement of facts showing the demand or need for the proposed service;

    (5)

    A description of each vehicle to be used in the applicant's operations, including the make, model, mileage, all vehicle identification and registration numbers;

    (6)

    Verification that all of the staffing requirements of the board, and of state law, will be met. Such verification shall require submittal of the names, addresses, and certification of all personnel to be employed;

    (7)

    The address and description of each of the locations from which the applicant will operate, and the hours of operation and staffing that is proposed for each location; including the applicant's present and any proposed base station location and all substations;

    (8)

    A complete description of applicant's proposed dispatch, recordkeeping and reporting system capable of documenting unusual system overload, and of identifying calls subject to an exemption from response time requirements. A complete description of the applicant's communication system, including its assigned frequency, call numbers, mobiles, portables, range and hospital communications ability;

    (9)

    A proposed schedule of rates, fares and charges;

    (10)

    A drug-free workplace certification;

    (11)

    A compilation statement showing assets and liabilities prepared by a certified public accountant;

    (12)

    A list of equipment and supplies which will be routinely carried on each vehicle;

    (13)

    County reserves the right to inspect the applicant's workplace, and/or vehicles for compliance for county or state requirements;

    (14)

    A sworn statement signed by the applicant or an authorized representative, stating that all the information provided by the applicant in the application is true and correct; and

    (15)

    Any other information as may be reasonably required by the board or may be required by the county administrator or designee shall be provided by applicant.
